Incident Reporting: A Critical Process for Organizational Safety Importance of Incident Reporting Organizational safety is paramount, and incident reporting is a crucial process in maintaining a safe work environment. Effective incident reporting allows E Light to learn from their mistakes, identify areas for improvement, and prevent future incidents from occurring. By reviewing and analyzing every near miss, incident, and accident, every one of us at E Light can understand what we did right, what we could have done better, and what we did wrong, ultimately enabling us to repeat successful practices and avoid pitfalls. Incident reporting is a collaborative effort, and the success of the process depends on the participation of all employees. We have designed a smooth and efficient reporting system that encourages employees to come forward with information. We have established a clear notification process, designed a single point of contact, that involves all the necessary stakeholders to provide an effective notification and follow up system. Incident Reporting Procedures When a near miss, incident, or accident occurs, it is critical that the Director of Education and Loss Prevention, Ted Smith, be notified immediately with a simple phone call. If the Director does not answer, the employee should hang up and immediately call again, as this will signal the urgency of the situation and prompt the Director to take the call and respond. Once notified, the Director will then take several actions to ensure the appropriate response. First, the Director will advise of any action need to take right away and let you get back to it. Second the Director will contact the supervisor, project manager, department head, and the company President to inform them of the situation. The Director will also notify the safety department, who will then directly engage with the employee to assist in addressing the incident. This comprehensive notification process ensures that the necessary steps are taken to address the situation, while also keeping the relevant stakeholders informed. This process also allows you to deal with the situation at hand and to do so with support. You don’t have to worry about making other notifications, knowing what exactly to do or what processes to follow. You will have a team of people working with you to help. This process will also avoid tying up communication lines by avoiding having multiple people calling each other. By following this clear and streamlined incident reporting procedure, employees can focus on the immediate response, while the organization's leadership and safety team can work together to investigate the incident, identify root causes, and implement corrective actions. Overcoming Barriers to Incident Reporting Despite the clear importance of incident reporting, there can be barriers that prevent employees from coming forward. You may be worried about negative responses from management when reporting a near miss, incident or accident. The only negative response that could come from management concerning reporting a near miss, incident or accident is if you fail to report the near miss, incident or accident and that failure to report causes confusion, bad communication, incorrect response or inadequate follow up. Conclusion Incident reporting is a critical component of our safety. By following this consistent and well-defined reporting process, E Light can effectively gather the information needed to understand and address safety issues, ultimately leading to a safer work environment for all of us and we can efficiently respond quickly to situations.

The primary purpose of this new procedure is to ensure that the vehicle is in a safe and roadworthy condition before the driver operates it, while also serving to focus the driver's mindset on the task at hand - safe driving. Research has shown that a structured vehicle inspection process can significantly contribute to identifying potential hazards and improving overall safety outcomes, underscoring the importance of proactive measures like the Walk-Around Pre-Use Inspection to mitigate the risk of accidents on the road . The monthly vehicle inspection report will still be required. Sunday, August 04, 2024 Fatigue is a multifaceted experience that can manifest in distinct ways depending on the nature of one's work. On one hand, there is the weary exhaustion felt after a day of dealing with others' issues and emergencies, leaving one uncertain of their own accomplishments. On the other hand, there is the satisfying tiredness that comes from a productive day, where one can reflect on their tangible achievements as they wind down for the night. The construction industry exemplifies this duality, where a wide range of interdependent tasks and roles must be seamlessly coordinated to bring a project to fruition. Designers, estimators, superintendents, prefab departments, education teams, accounting personnel, HR, loss prevention, and tradespeople all contribute to the complex, overlapping process. While some tasks may be more physically or mentally demanding, each component is essential to the project's success. Recognizing the importance of even seemingly mundane or tedious responsibilities is crucial, as the failure to complete any one aspect can jeopardize the entire endeavor. In my experience in the industry, I have learned four valuable things that keep me motivated and always moving forward: 1. Always remember the big picture and what we are trying to accomplish overall. 2. Keep in mind that every part of the process is important and critical to the process. Even sweeping the floors, organizing material, digging ditches, it doesn’t matter, they are all important. 3. Try to do every task safer, better and faster than anyone else. Make it a competition, even if it is only challenging yourself. 4. Learn everything can about the industry, from every source you can. Knowledge is power and the more you learn the better prepared you are for anything. If you can approach each task with this mindset, you will not only be successful as an electrician, but you will also love doing it. Let’s all remember we are part of a championship team and let's strive every day to do everything safely, efficiently, and the best we can. That way as lay in bed ready to sleep, we can say…. If you are going to be late or absent, you must call, NOT TEXT, Joe Madrid to let him know. 719-640-1876. If you are not ready to work at start time when Mikayla starts the Stretch and Flex, you will not be allowed to work unless you called Joe Madrid and let him know you were running late. If you do not call Joe Madrid, an absence will be considered a No Call No Show. At the end of the shift, before leaving, each employee to check to make sure all trash cans are empty, every tool is put away, trash cans are inside, pallet jacks are inside, pallets are stacked neatly, trash is picked up off the floor, and trash is picked up out back. A disorganized storage system and material area is a sign of disorganized shop and work force. Every employee is responsible for keeping everything organized and neat. We will not have a disorganized work force. If you are not a neat and organized person and you cannot keep work areas and storage areas neat and organized, this is not the organization for you. Report ANY unsafe conditions to your supervisor. We will adhere to all company & safety policies, also OSHA Standards for temporary electric & general safety requirements. Report ALL injuries to your FOREMAN or project supervisor IMMEDIATELY. Housekeeping/clean-up will be required by ALL employees on a daily basis. Everything must be returned to its place and floors swept and the outside entrance area swept. The printer and large screen must be covered, and all tables cleared and organized. Each employee will have a current pre-task card on their person at all times while on duty and the corporate challenging policy will be followed. At the end of each shift or task, all trash must be picked up and thrown away, all tools must be returned to their case or their shadow wall. NO EXCEPTIONS. All floors swept, all trash cans emptied and the back area picked up and swept up. The kitchen wiped down and the coffee pots turned off and placed in the dishwasher and the dishwasher ran. All tools must be returned to their storage location and put back in their cases. Safety glasses and warning vests are to be worn at all times on the job site. 100% natural fiber clothing is recommended, and sleeves must go to at least the elbow. Footwear must be sturdy work boots. Your personal tools are not covered by E Light's insurance; we recommend you take your personal tools home with you each night. All company-supplied tools need to be returned to carrying cases before being placed in the gang box for storage each day. Gang boxes need to be kept closed and unused tools put away. Stretch and Flex shall be conducted before each work shift. There is a place for everything and everything has its place. You are responsible for putting everything back in its place when you are done with it. Do not leave empty or partially full bottles of water laying around.
