Please fill out all the requested information if you have deliveries coming to prefab that are going to be direct shipped or if you need prefab to pick up items from a third party for delivery. If a field does not apply to your delivery please mark it as N/A. When you have completed the process please click Submit. Do not use this form for Tool Transfers and Requests. Those requests are to be done using the iAuditor form and submittal process. Thank you

This form will be submitted directly to the Logistics Manager, the Prefab Driver, the Prefab Manager and the Director of Education and Loss Prevention. The information will then be processed and tracked. The Logisitics manager will call you if they have any questions.
