Request for Leave

Communication is important to success so before filling out this form, you must directly communicate with your immediate supervisor first. You must also use this process to communicate your Request for Leave to Human Resources and Accounting to ensure that your Request for Leave is reviewed and processed. 

 

 Requests for vacation pay must be made using Workmax; this form DOES NOT initiate vacation pay.

Requests for vacation pay must be made on a timesheet; this form DOES NOT initiate vacation pay. 

What is the purpose of this form?

​

1. To notify in writing and to obtain your supervisor's written approval for the leave.

​

2. To notify Human Resources of your leave. HR will add your name to the leave calendar which can be accessed by management and supervisors. 

​

You will need to process your leave pay, if applicable, on the week of your leave using Workmax in order to receive payment for the leave.  If you are not sure how to process your time for leave in Workmax, check the How-to Page or call Human Resouces and ask for guidance.
