Accident and Incident Reporting

Accidents and incidents are unfortunate events that can and do occur. It's how we respond to them that matters. At E Light, we prioritize learning from these events to prevent their recurrence. Our Safety, Health and Environmental Policy (SHEP) outlines our commitment to immediate reporting, thorough investigation, and continuous improvement. Understanding the difference between an accident and an incident is crucial. An accident results in injury or property damage, while an incident has the potential to cause harm but does not result in damage. Both require immediate reporting and investigation. Upon occurrence of an accident or incident, the Director of Education and Loss Prevention must be notified immediately. This ensures a prompt and appropriate response. Investigations must be detailed, with a focus on root cause analysis. Legal, insurance, and client obligations necessitate timely reporting. Our policy mandates that investigations commence immediately, with a final report submitted within 8 hours. If this isn't possible, a preliminary report is due within 8 hours, followed by a final report within 24 hours. The SHEP is a comprehensive document, available on our website. Familiarize yourself with the Reporting and Investigating section to understand your role in the process. We don't expect you to memorize the SHEP but we do expect you to go to it on elightinformation.com and look up things as you need them. The idea is that you have access to the policies and procedures at your fingertips, please utilize this powerful tool as things happen or when you are unsure of the process or policy. All incident and accident reports must be reviewed and signed by the Director of Education and Loss Prevention and the President before release to clients or agencies. This ensures we've identified root causes and developed a plan to prevent future occurrences. Let's work together to create a safer, more proactive work environment.
